Application
Completed application should be mailed to the internship director, postmarked by
February 15, 2008. The Appointments will be made in April for entrance in August.
All of the materials listed below will be included for consideration of admission:
- Letter of application: two pages or less including, but not limited to, short term and long term professional goals,
reasons for choosing dietetics as a career, expectations related to the internship, and the areas of strength
which would make the applicant a particularly successful candidate.
- Completed 2005
Supervised Practice Program Application.
- Three Recommendation Forms (one must be from a former professor or academic advisor).
- Official transcripts from each college or university attended.
- Verification form or Intent to Complete form from the undergraduate DPD.
- A non-refundable application fee of $45.
- All applicants to the dietetic internship must participate in the computer matching set up by the ADA and D&D

- A degree or non-degree application to the graduate school at Southeast
Missouri State University must be completed and included in the application
packet. A non-degree application for admission www2.semo.edu/gradschool/app_forms_curr.html
should be included with materials submitted to the Internship Director by
those applying to the internship only. Submit a check for $25 payable to
"Southeast Missouri State University". Students interested in completing
an M.A. in Human Environmental Studies or an M.S. in Nutrition and
Exercise Science and students interested in receiving financial aid must
complete a regular admission application www2.semo.edu/gradschool/app_forms_curr.html and submit a $25 check
payable to "Southeast Missouri State University". This fee will be returned to students who do not
match at this university. Acceptance in the internship does not guarantee acceptance in the program in
Nutrition and Exercise Science. Minimum requirements for regular acceptance into the Nutrition and Exercise
Science program are a minimum 3.0 GPA and 1500 on the GRE.
- International students are required to submit an "International Student Application" to the graduate school.

$100 made payable to Southeast Missouri State University. Include both the graduate school
application and the check with your application to the Dietetic Internship.
